WHERE ARE YOU LOCATED? 

Initial appointment and follow up visits are held in my new home office in Harrisonburg. Once an appointment is booked, client should receive an email with specific instructions for visit and the detailed address information.

*Note: At this time, I cannot offer handicap access as there are 3 steps to the front door. Please let me know if this is an issue as other arrangements will be offered.

do you accept insurance?

At this time, I am able to provide each client with an itemized bill (super bill) that is ready to be sent to client’s insurance company for reimbursement, if client’s insurance plan covers nutrition therapy. You should always check with your insurance if MNT is part of your benefits. A notification will be sent to all my clients if there are changes to this.

what is your Payment policy?

Clients are expected to pay full price of visit at the time of visit or prior. No payment plans available at this time. Contact me for more information on sliding-scale payments if necessary.

what is your cancellation policy?

Clients will be charged full price of visit if appointment is not cancelled within 24hrs.  A credit card may be requested to hold appointment.

what happens once I book my appointment?

An email will be sent to client with an “opt int” request from Kalix Health, the electronic medical record company I use. Once you opt in to receive emails from Kalix (no spam will ever be sent), Kalix will send you the Appointment Letter (or confirmation of appointment) with specific instructions on location and what to bring to first visit as well as my Practice Policy. Then you will receive my New Client Intake Form to be completed prior to first appointment. All of these forms are meant to be completed electronically.
